Sexual Misconduct and Gender-Based Violence Report Form

This form may be used to report any incident(s) of potential violation of the Bowdoin College Title IX Policy or any other incident(s) of sexual misconduct or gender-based violence at Bowdoin. All reports are received by the Title IX Coordinator and will be reviewed within one (1) business day.

If this is an emergency or if you require immediate assistance, please call 9-1-1- or contact the Office of Safety & Security at 207-725-3500.

Please complete this form with any information you are willing and able to share. Because the College may have an obligation to address certain reports, we cannot guarantee complete confidentiality where it would conflict with Bowdoin's obligation to investigate meaningfully or, where warranted, take corrective action.

You may choose to remain anonymous. If you do not provide contact information, please be aware that the report will be used for statistical purposes.

In consultation with the Title IX Office, the Executive Director of Safety and Security will determine if the anonymous report includes sufficient information to be logged or reported for Clery statistical purposes.

Retaliation for filing an incident report is strictly prohibited.
